Preparing for your Appointment
A requisition form is needed from your doctor for you to get an ultrasound appointment in our facility.

To schedule your visit smoothly, please keep the following documents handy and take note of the information before coming in for your appointment:

A valid health card;

A piece of identification e.g. driver's license;

Bring your requisition if you have it with you. Sometimes your doctor may have sent it by fax;

Arrive 15 minutes before your appointment;

Please call to cancel if you are unable to keep your appointment;

We will allow only one support person or family member during the exam. Please arrange to leave young children at home or have a support person to supervise children in the waiting room during your exam.
